HOA & FSM Escalation Request Form

Please only use this escalation form for matters regarding home inspection timelines.It is your responsibility to contact the Field Service Manager (FSM) regarding these issues, and ensuring that all utility activation, deactivation and when applicable, re-winterization payments are made. FSM contact information for each case is provided on the cover letter of your contract package. Please note that the inspection period is 15 days from the time the contract is executed and failure to submit necessary forms to the FSM could delay this process. Please submit your escalation request here only after you have contacted the FSM so that MMREM may assist in getting this completed for you. If you submit an escalation request that does not concern home inspections, your request will not be reviewed in this forum.
